+ Do I need to have a referral for my child to be seen?
Legally, therapists at CDOT can treat your child without a referral from your doctor.
If we are billing your insurance, it is the insurance company that may require the referral. To absolutely ensure your claims are paid by insurance without any challenges we ask all families to get a referral.
Even though every patient is required to have a referral, there does not need to be a specific diagnosis to come to CDOT! A medical doctor, nurse practitioner or psychiatrist may sign the referral.
+ How do I know if therapy will be covered by insurance?
We are in network with Blue Cross Blue Shield of Illinois! Depending on the plan or policy that was chosen will depend on the cost of therapy. We ask you to reach out directly to the insurance company to better understand your benefits.
+ What do I need to do to schedule an appointment or evaluation?

+ I had an evaluation somewhere else. Can I come to CDOT?
Yes! Most medical evaluations are good for a year! We can accept evaluations from other facilities! We would need a copy of the evaluation before we schedule to insure billing and scheduling parameters are correct.
+ What if I can’t pay for therapy?
CDOT has many alternative funding sources that can help to pay for therapy. Please reach out so we can help you.
+ What are deductibles, copays, and coinsurances?
Deductible: a specified amount of money that the family must pay before an insurance company will pay a claim
Copay: a set amount to be paid by the family per date of service
Coinsurance: a percentage to be paid by the family per date of service
+ Do you accept school evaluations?
Unfortunately no. School evaluations are not medical evaluation and can not be used to bill insurance. However, the information may be useful for the therapist during treatment!
