Talk with your Pediatrician
Discuss your observations with your pediatrician and why you think that your child could benefit from occupational therapy services. Pediatricians can provide referrals for occupational therapy services.
What to bring to your first appointment:
Insurance Card
Please bring your insurance card with you to your first in-person appointment.
An Understanding of your Child’s Insurance Benefits
If you have any questions about your insurance coverage, please feel free to contact us.
Completed Paperwork
Paperwork can be printed, completed, and brought to your first appointment with us. You can also complete all paperwork via the Client Portal:
Copay
CDOT is able to process copays by payment via cash, card, FSA, or HSA accounts.
The Evaluation Process
Your family is our first priority. We want to know the parents’ concerns and the child’s concerns.
During your first visit you will have the opportunity to talk through your concerns with your therapist and determine a unique treatment plan designed specifically for your child’s needs!
To establish care, give us a call or send us an email. We can then help you secure a referral from the pediatrician, if you don’t have one already!
After the referral and insurance information is received, we will call you to schedule an evaluation.
We will then send you a welcome email with any paperwork that would need to be filled out prior to the appointment!
